The Problem:

To create an HL7 based Healthcare ecosystem that will connect to the State HIE's Connect system.  This system has to be an open source system that is capable of being implemented to existing Epic based systems, for State HIE member Hospitals, mid/small sized Clinics and single Doctor's practices.  System must interface with SHARPS and Consent2Share systems. Integrate Patient and Data/Segmentation Security and Privacy Protocols with SAMAHS and create searchable Audit Trails.

The Solution:

A healthcare ecosystem that is made up of applications from healthcare vendors who have created systems; that have been used in healthcare systems and have proven that their systems can work with the Epic HL7 systems; and open source to work with ILHIE and future HIE's around the country.

Specific to the ILHIE, the healthcare ecosystems must address the following requirements:

I have identified three companies that can work together to create an ecosystem that can work with  Epic systems, who has been tapped to create the ILHIE  Master Patient Index, and that will link Illinois  healthcare systems together through a system called ILHIE Connect.

System Workflow

How difficult is the approach to implement and administer?

7 Medical systems is based on the HL7 standards and can integrate with existing EMR with or without HL7 system. Since 7 Medical can integrate with HL7 standards, it can be used with ILHIE Connect interface requirements. 7 Medical also integrates with Epic systems software that is the basis of the ILHIE Master Patient Index system.

AuthentaScripts is an open source node system that can work with existing EMR systems and has interoperability with e-prescription systems. These nodes are then sent to a system of Broadcasters that can integrate with Doctors, Pharmacies, and Insurance Providers. Since AuthentaScripts is an open node system it can integrate with ILHIE Connect IT requirements.

What is the current consumer experience?

7 Medical is a web-based system that can be accessed by a doctor or staff. 7 Medical has a patient portal that can be set up in the future to allow patient access to their medical records via desktop or mobile devices.  ILHIE Connect users can integrate ILHIE web based messaging systems to 7 Medial portal systems.

whosPOPpin is a web-based system that does not require users to download an app to a mobile device. The system can be easily integrated into any EMR doctor or patient portal with minimal training. The system can also integrate with Skype and can be used in telehealth systems. ILHIE Connect users can use whosPOPpin systems to integrate into the ILHIE messaging systems and future ILHIE Connect Patient Portal system.

What is the governance process?

7 Medical

Independent auditor verification that 7 Medical has internal HIPAA privacy and security controls in place.  The examination was performed by applying HIPAA Administration Simplification, Part 164 Security and Privacy Standards and, without exception, 7 Medical's internal privacy policies and controls comply with these HIPAA standards. These security standards are also part of the ILHIE Master Patient Index and ILHIE Connect requirements.

AuthentaScripts

1. A Node (node #n) is installed and initialized for the first time. It sends a ping

request to Broadcaster.

2. Broadcaster receives an anonymous request to authorize from an unidentified

Node and logs the request for approval by Administrator.

3. Administrator reviews the Node details and approves or denies the request.

4. If denied, Broadcaster ignores any new future requests from that particular

Node.

5. If approved, a unique API key (API Key #n) is issued to the requesting node (node #n).

6. Node #n uses the ‘API Key #n’ to authenticate itself.

7. The broadcaster validates the login request and provides a set of rules for that particular node. These rules contain access policies, polling & communication rules, triggers and data to capture from the target system.

8. Node receives the rules and starts processing them and communicates the output back to the Broadcaster. The AuthentaScripts systems can be installed behind the Firewall of the ILHIE  Connect  systems and be used to security ping the database and send an audit of any request made to the data base.

whosPOPpin

Web-based system sits within the Firewall and, as bi-directional security that connects with target system, with a secure certificate that send a password API to the systems administrator. The system then checks the request against system policy. Once certificate is authenticated the user is allowed access.  When the session is over the certificate is disconnected with no information left on the web connected device. This complies with the ILHIE Connect Integrated Data/Patient Segmentation system.

Is it practical?

7 Medical

7 Medical systems EMR systems can be designed to create specific patient information from lab reports, pharmacy, examinations, scheduling , etc. 7 Medical also prevents duplication of data and only gets the patient data needed based on specialty.  7 Medical also uses HL7 protocols that can integrate between two different systems and using policy subsets to redact information from that subset, meaning that 7 Medical could be used in the Data Segmentation and Privacy requirements currently being developed by the ILHIE.

What is the impact on clinical workflow?

This system can be setup to work with all types of clinical workflow from patient diagnosis, scheduling, e-prescribing, and network with existing legacy systems. 7 Medical systems works with the Epic system and can integrate with the ILHIE Patient Data/Segmentation systems and is flexible enough to be updated based on legal policy that restricts sharing of subsets of patient data.

AuthentaScripts

Broadcaster and node systems can be configured to work with legacy systems or set up new systems even if healthcare provider does not have an EMR.  AuthentaScripts can set up to send only patient sub set information about the patient prescription based on ILHIE Patient Data/Segmentation systems.

What capacity do EMR and HIE partners have to implement?

7 Medical

EMR systems can be deployed based on the providers specialty and can be secured via a Firewall or secure cloud system. 7 Medical has SAS 70 and ONC certification and conforms with Meaningful Use 2+ policies. ILHIE partners can use 7 Medical to create a new network or integrate with their existing policy and create restricted data flows based on these policies.

AuthentaScripts

Broadcaster and Node system can be deployed to send and receive requests from a doctor to the pharmacies with restricted data to the pharmacies that only have the patient data needed to fill that prescription. AuthentaScripts can be integrated with existing e-prescriptions systems to follow and send alerts to the doctor when someone pings the patient information and sends alerts according to the policy set up by the EMR system.

Possibility of phases / iterative process?

7 Medical and AuthentaScripts can create SQL based data-bases that can then be used to set up to the proposed ILHIE  Master Patient Index that is scheduled to be set up in the future.
